L’impacte matern davant el naixement prematur i la influència en el desenvolupament socio-emocional de l’infant: revisió sistemàtica

Resum:

Introducció: A l’actualitat el nombre de naixements prematurs es molt elevat i les condicions estan canviant, la taxa de supervivència es cada vegada més alta, les setmanes de gestació més baixes i per tant l’impacte de la prematuritat més evident. Les pràctiques professionals dutes a terme a les Unitats de Cures Intensives Neonatals també han evolucionat i actualitzat els seus mètodes amb enfocs centrats en el desenvolupament de l’infant de forma integral i centrades en la família. A més, es recull evidència que el naixement prematur causa un impacte en el benestar emocional i mental de la mare. Objectiu: En la present revisió sistemàtica volem conèixer com l’estat de benestar emocional i la Salut Mental de la mare d’un infant prematur, causa interferència en la relació primerenca entre mare i fill i el posterior desenvolupament socioemocional de l’infant. Mètode: S’han seleccionat 10 articles extrets de les bases de dades Psycinfo, Scorpus, Web of Science i Pubmed, i s’han revisat seguint el protocol revisat basat en la guia metodològica PRISMA. La recerca es va dur a terme per resum d’articles i sense cap restricció temporal. Resultats: Els resultats principals trobats indiquen que les mares d’infants grans i extrems prematurs presenten un baix benestar emocional, símptomes d’estrès post-traumàtic, ansietat i depressió. Aquest estat emocional i mental, interfereix en establir relacions primerenques basades en la interpretació sensible, qualitat relacional i plena disposició emocional per respondre a les necessitats afectives de l’infant. Conclusions: L’estudi realitzat ens permet considerar la variable materna com una peça fonamental al desenvolupament saludable i harmoniós de l’infant. Introduction: Currently, the number of premature births is very high, and conditions are changing. Survival rates are increasing, gestational weeks are decreasing, and thus the impact of prematurity is becoming more evident. Professional practices carried out in Neonatal Intensive Care Units have also evolved and updated their methods with approaches focused on the integral development of the child and family-centered care. Additionally, evidence shows that premature birth causes an impact on the emotional and mental well-being of the mother. Objective: In this systematic review, we aim to understand how the emotional well-being and mental health of the mother of a premature infant interfere with the early relationship between the mother and the child and the subsequent socioemotional development of the child. Method: Ten articles were selected from the databases Psycinfo, Scopus, Web of Science, and PubMed, and were reviewed following the revised protocol based on the PRISMA methodological guide. The research was conducted through article summaries without any temporal restriction. Results: The main results suggest that mothers of very and extremely preterm infants deal with low emotional well-being and symptoms of post-traumatic stress, anxiety, and depression and these are of significant relevance. This emotional and mental state interferes with establishing early relationships based on sensitive interpretation, a quality relationship, and full emotional availability to respond to the emotional needs of the infant. Conclusions: The study conducted allows us to consider the maternal variable as a fundamental piece in the healthy and harmonious development of the child.
