Análisis de las actitudes hacia la propia muerte en las cuidadoras familiares de pacientes en situación de final de vida. Yo después de ti (Me After You)

Abstract

El individuo, en situación de final de vida, presenta un declive funcional, y en su entorno existe una cuidadora como pieza central de los cuidados. Esta no solo debe asumir esa carga de la dependencia, sino que precisa anticiparse y adaptarse a las transiciones futuras. Se ha estudiado cómo la carga y las circunstancias del fallecimiento afectan a la cuidadora, su entorno y a los profesionales sanitarios. Pero existe poca investigación del proceso de transformación interno que acontece durante la elaboración del duelo, donde puede existir una reflexión hacia su propia muerte. Por ello, se plantea investigar sobre el proceso de transformación interno que existe tras el contacto con la muerte de un familiar. Se plantea una investigación cualitativa de tipo fenomenológico exploratorio en el entorno del barrio de Besòs (Barcelona). A través de un muestreo teórico se seleccionarán personas que hayan cuidado a familiares en final de vida según su género, grado de parentesco y las circunstancias de final de vida del paciente cuidado. Se realizarán entrevistas semiestructuradas por parte del equipo investigador que posteriormente realizará un análisis de contenido temático del corpus textual construido con las transcripciones de las mismas. Con ello se obtendrá un marco interpretativo del fenómeno que será sometido a validación por el equipo investigador y los informantes. Poder recoger el testimonio sobre los actos de cuidar a pacientes en final de vida permitirá conocer este fenómeno, así como enfocar intervenciones que permitan trabajar y consensuar en cuidadoras los deseos sobre la atención sanitaria en final de vida.

The individual, at the end of life presents a functional decline, and in their environment there is a caregiver as the centerpiece of care. Not only do that person accepts that burden, but she also needs to anticipate and adapt to future transitions. Thus, the impact of how the burden and circumstances of death affect the caregiver, her environment and health professionals has been studied. But there is little investigation on the internal transformation process that occurs during mourning, where they may think about their own death. Therefore, it is proposed to investigate the internal transformation process that exists after contact with the death of a relative. A phenomenological exploratory qualitative research is proposed in the surroundings of Besòs (Barcelona). Through a theoretical sampling, people who have cared for family members at the end of life will be selected according to their gender, degree of kinship and the circumstances at the end of life of the patient being cared for. Semi-structured interviews will be carried out by the research team, which will subsequently be analyzed on the thematic content. With this, a narrative framework of the phenomenon will be obtained that will be validated by the research team and the informants. Thus, being able to collect testimony about the act of caring for patients at the end of life will allow us to know this phenomenon as well as focus interventions on other caregivers and let those express wishes about their health care at the end of life.
